Forms and video certificates that are required to participate in any school workout

Please remember that any winter sport athletes who want to workout with coach Druehl or coach Caron must have all paperwork in. 
Please also check your emails for an email from Amanda Zamani in regards to baseline concussion testing. 
 
 
Instructions for Ransom Everglades Student-athlete Clearance:
Please begin by downloading a copy of the FHSAA EL-2 to be completed by your child’s physician. This form will need to be uploaded during the clearance process outlined below.
Your child will also need to complete the following NFHS/ FHSAA courses AND upload the completion certificates. Instructions regarding form completion are here.  


FHSAA Clearance Registration Process
Create your account 
Create an account by clicking on the “register” link next to “Don’t have an account?” then provide a valid email address, password and answer to a security question. Save login information for future years.

Click on the “New Clearance” button.
Enter in information requested for your student-athlete starting with the year (2020-21), school and sport. You will be able to add additional sports later in the process. Save your information after each step in the process. You will be asked to enter; 
Step 1 Student Information - will be retained for future years 
In this section you will be asked to provide a student ID number. Enter your seven-digit Ransom Everglades School ID number which can be found on your child’s ID card or on MyCOMPASS by clicking on your child’s “contact card” and then looking in the “general information” section.
You will be asked to upload the completed physical form (EL-2) and the NFHS/ FHSAA video certificates in this section. 
The physical form is to be filled out by the physician who is conducting the physical. Physical forms are valid for 1 full year from the date of the exam. (Example: doctors’ physical exam performed on May 22, 2019, expires on May 22, 2020.)



Once completed, scan the physical form (EL-2) and upload it to your clearance account. You must also upload a copy of all three completed NFHS course certificates:
    1. concussion
    2. sudden cardiac arrest 
    3. heat illness
If you do not have a scanner, you may simply take a picture of these items with your phone and email or save it to your computer and upload that way.
Step 2 Medical Information 
Step 3  Parent/ Guardian - will be retained for additional children and future years 
Step 4  Signatures - parent AND student 
You, as the parent/guardian, will need to read the form(s) (located on the right side of the screen) and type in your name on the line requesting an online signature indicating that you have read, understood, and will adhere to the corresponding text. There will be a section for the parent/guardian to fill out and also a section for the student. Make sure that you go over them with your student present and have him/her sign their section as well. These are the FHSAA EL-3 and Ransom Everglades forms.

Your child is NOT CLEARED to participate until all of the above steps are completed AND verified by the school and you receive an email indicating the student athlete is cleared.
